Sent Multi-selected Items From Selectbox To Textbox

Example:- As in phpmyadmin
if u click on SQL, here comes a textarea and a multi-selectbox(Fields) and a button as (<<) in between them. we select items from selection box and click this (<<) button and values go to textarea with comma separated.
i am currently creating an order form. In this form the user has a lot of different choices to make, so I use select boxes to show them the most common ones, but these are just "suggestions", the user should not be limited to these options. Just take a look at this example:

This works as it should, if "Other" is selected, the <select>-Node is removed and is replaced by a text field with the same name. (This is important because the server side script is already finished and just takes these values.) However, if "Other" is selected but the textfield remains empty after it lost focus, the original select box should be displayed again. Regarding this removing the HTML from the DOM doesn't seem very clever, but I doubt it's a good idea to have 2 elements with the same name (select box and textfield). Any ideas how I could get this working?
have a text box which when searched with 3 characters of vendor name will do ajax request and populate the results below the textbox, beside the box are two arrows pointing to a selectbox, when i clcik on the right arrow after the search the vendor name should to added to the select box....similarly after i add required vendor names, I should be able to delete the name from the select box.I have completed the ajax search part of the textbox, and I am stuck with the passing value to the selectbox portion.
I have a multiselet box in which i identify the selected items. Once the user has finished selecting the items it causes the form to be submitted. For this example it just shows the selected indexes. To see the problem. Copy paste the code and save as html file. Then click on an item in the select box.

So far i managed to show/hide (the only) 1 textarea if an option is selected, using javascript. How can i show all textareas in form if an option is selected (lets say <option name="active">) and hide if option selected is name="passive"?
I have two selectboxes. I just want it something like this, when I select a value in my first selectbox the second selectbox will show. And when I select different value on my first selectbox, my second selectbox will hide. Meaning, there is one value in my first selectbox that has the ability to show the second selectbox.

I have a form for taking customer details. Part of the form will allow the user to select some upgrades for their selected product. The upgrades will be displayed in a drop down list with their additional cost. The total cost of the product being ordered will be displayed on the page.
Could someone please advise me on the best way to automatically update the total cost displayed on the page each time an upgrade is selected? I am using PHP for the site but I'm assuming that javascript is the best choice for this.

i'm after some script to display some html depending on what is selected in a list box the html i want to display are checkboxes so it's more stuff for a form i'm using lotus notes, so i have to use javascript.
i can get it to display a checkbox if a particular item is selected but it won't show up in place, it loads a new page with the check box the only thing on that new page.

I am trying to create an order with the ability to dynamically self total the sum of the selected items but also be able to add a 25% labor fee having it be at least $90.So if someone buys $300 worth of items the labor charge would be $75 but it would be automatically bumped to $90.heres the existing code:

Is there a way to make a form label change color when, eg: a checkbox is checked, a menu/list item selected, or a radio button clicked?
Let say the label was dark gray and then will change to green when it is selected.
The bad news is I do not have the slightest clue as to how this is going to happen or if it is possible with Javascript. I searched Google but nothing Is this possible?
so what i've did is to check the onkeydown and check if they are digits and if input.value text length is 3 or more i will cancel it.However, if the user had words selected in the textbox, i will wish to allow input. Because imagine the user selected the string "123" in the textbox, and wishes to override this string by typing a new number. but with my current logic, since the textbox already have 3 digits, he had his input rejected, which is not what i would want.
My code (below) works if I place the selectBoxes in different cells on a table. However, what I want is only ONE selectbox displayed at a time, not the 3 different select boxes in 3 different table cells of the table.
I initially want the citySelect to display, but if user clicks option [-- change country --], then the citySelect is REPLACED by the countrySelect. Then, on making a country selection, the countrySelect is REPLACED by a new citySelect which is populated with the new city list.

I have three radio buttons and one textbox. If user select one of those radio buttons and type something in the textbox which is the value of the textbox, I want that value to be the value of the radio button that is selected, now the user may enter the value into the text box and then select the radio button or first select a radio button then enter something to the textbox, anyway the value of radio button should be equal to textbox value that user enter!
