For some security reasons I would like to show an email field in a form ONLY if the 4 checkbox are checked.I don't want to use server side coding so I need to use javascript.I have this code, showing an alert if the 4 checkboxes are checked:

I have a drop down selection where it shows and hides form inputs depending on selection. When the page loads it shows the drop down selections with no form inputs. After user selects drop down it shows the form fields. It works great but I want it to show the same form fields after the user hits the submit button which submits the page to itself. It currenlty goes back to the default selection list and doesnt show any of the form fields. Basically it hides all the form fields after the user hits submit button. How do I make it keep the last shown form fields after the user hits the submit button?

I'm trying to get the two followup questions underneath the checkbox to show up only if someone places a check there, but for some reason the way I've got it set up now it's simply hiding the area I want to show up altogether, and the checkbox has no effect on it.

It works fine in IE, but in Firefox it appears to fail on the eval lines.
I've searched around but I can't seem to find the correct invocation to get

I'd like the user to select 3 variables (radio buttons would also work) - whether the victim is Indian/Non, the perpetrator is Indian/Non, and whether the crime occurred On/Off a reservation and have a div that displays the possible jurisdiction outcomes based on the selections.For instance, if the user selects Victim: Non-Indian, Perp: Indian, Location: On reservation, the possible jurisdiction results would appear.

I have a form with a field that asks for a full year. If the year is greater than 20 years ago, I'd like additional form fields to be revealed. If the year is equal to or less than 20 years ago, it will not reveal the fields. (And it should be based on getFullYear(), so I don't have to update the script every year, yeah?)
I figured out how to script onclick functions and some basic .value == functions in order to show/hide just about every type of form field. So I don't need that info. But I'm unsure of how to calculate the value.

I am having some jQuery troubles whereby I have some jQuery that toggles an 'Other' HTML input field and associated label when a user selects the value Other from a HTML select drop down. I have this working for one field but the application I am building has increased in scope whereby there may be multiple instances of members on one page so the Other option will be there multiple times. At the moment if a user selects Other from one drop down all Other input fields show. How do I make this exclusive without repeating the jQuery for the separate instances?

I have a form that i wish the entered fields to be emailed to 2 addresses; my address (bcc) and the email address that the user has entered in the email field on the form. Before you say it, I know that 'mailto' is not a recommended method but it the only one I can use (due to restrictions at work). Below is the code I am using but it doesn't seem to like [document.forms.request.email.value]. I know it works (including the validation) when I just enter an email address in the code but it doesn't using the email form field.

The script works great, BUT I need to use it in 2 different areas of the same form. It only works on an id="selectMenu" and of course I cannot have more that 1 id.
How can I get this script to work in multiple select fields? (at least more than 1)
I am trying to validate a web form. Someone else wrote the Html code and implemented it with an iframe using asp and php. I didn't want to mess too much with the code so I decided to use javascript to make the form validate, before it was sending email alerts with every field blank. All works fine with that, however... I have created another page which retrieves the results for an individual's form submission, and I want to hide the sections that were not completed (hidden when submitted). How can I get my page to hide or show DIVs, depending on the value retrieved from a particular field within the database? i.e. if Question1Response="A", show Div ID="A Section" If Question1Response="B", show Div ID="B Section" etc.

However, this will work fine when calling with a link "javascript:validate('inputForm');" but if I try to prevent users by submitting form with pressing enter I put it in form onSubmit parameter: <form .. onSubmit="javascript:validate('inputForm');"> which will check the forms and submit data (do return) no matter if it matched or no.
I'm now working on kind of invoice form which in it the user can add as much input field as he wants.
The problem is that after adding a new input field - the content in the other fields is deleted.

I am building a Point of Sale system and I have a web form where the clerk enters the items being purchased... The product and the quantity. Then they click a button and an AJAX request adds the item to the sale in the database and fetches the price - the price is filled in, the total updated, and a new row is added to the form so they can enter the next item.
What I'd like is to that automatically when they tab out of the last field in the row to save time by not using the mouse... (tab and enter is a lot for some of my users)
What's the best way to do this? leave the buttons and have them activate on focus? an on blur on the last field?
